The ACA recognizes the unique relationship between the U.S. government and Native nations. As part of that commitment, American Indians and Alaska Natives receive special rights and benefits designed to make healthcare more accessible and affordable. If you qualify, you may be eligible for zero out-of-pocket costs — no copays, no deductibles, and no surprise bills for most covered services.


You can enroll or change plans any month of the year, and you’ll still be able to use your tribal or IHS services whenever you choose. This gives you the best of both worlds — access to trusted tribal healthcare and the flexibility of outside specialists or hospitals when needed.

 Not always. Many Native families qualify for very low monthly premiums (often under $50), but exact costs depend on income and household size. If your household income is at or below 300% of the Federal Poverty Level (FPL), you qualify for zero cost-sharing—no copays, deductibles, or coinsurance when using in-network providers. Premiums and cost-sharing are different: premiums are the monthly bill; cost-sharing is what you’d normally pay at the time of care. 


 It means your plan pays 100% of covered services when you use in-network providers (or approved Indian health providers), so you pay $0 in copays, deductibles, and coinsurance. You may still have a small monthly premium depending on income. 


 No. With an ACA Marketplace plan, you do not need tribal referral approval to see specialists or outside doctors. You choose your providers within the plan’s network, and the plan pays according to its benefits. 


 Yes. Marketplace coverage works with your IHS or tribal care, not against it. You keep access to your tribal clinic while gaining broader options (specialists, hospitals, urgent care) when needed. 


 The zero cost-sharing protections apply to eligible American Indians and Alaska Natives (AI/AN). However, anyone can apply for Marketplace coverage and may qualify for premium tax credits based on income—even if they are not Native. 


 Absolutely. We can help your entire household enroll on one application. Native members may receive Native-specific benefits; non-Native members can still qualify for premium tax credits based on household income. 


 For premium tax credits (lower monthly premiums), household income generally must be under 400% FPL. For zero cost-sharing, income must be at or below 300% FPL. Thresholds change slightly each year. As a 2025 reference, a family of four under about $124,800 may qualify for some level of help. 


 Yes. Many families use both. Insurance expands your options beyond the tribal system while you continue using IHS or tribal clinics for routine or convenient care. 


 Usually no. If your employer offers coverage that is “affordable” and meets minimum value, you’re typically not eligible for Marketplace premium tax credits or Native zero cost-sharing. If your employer plan is unaffordable or inadequate, exceptions may apply—ask us to review. 


 You can update your application any time. Your premium help or cost-sharing level can adjust accordingly. Native families also have year-round enrollment, so you can change plans monthly if needed. 


 American Indians and Alaska Natives can enroll or change plans any month of the year. You are not limited to the standard Open Enrollment window. 


 No. Enrolling in ACA coverage does not reduce or replace your tribal or IHS eligibility. It adds flexibility and can even support tribal systems by allowing insurance to pay when available. 


 

Yes. To receive Native-specific protections, you’ll provide proof such as a tribal enrollment card or official documentation from your tribe.


 Marketplace coverage travels with you. If you’re far from your tribal clinic or IHS facility, your plan’s network ensures access to doctors and hospitals wherever you live. 


 We verify eligibility, estimate your premium tax credit, ensure your Native cost-sharing benefits apply, help you choose the right plan and network, and support you year-round—not just during enrollment. 


 To receive ACA Native protections, you must be a member (or eligible dependent) of a federally recognized tribe. State-recognized tribes and heritage groups do not qualify for the zero cost-sharing benefit, though they can still get regular Marketplace subsidies if eligible by income. 


 Eligibility is based on tribal status and income, not the state. Some states use Healthcare.gov and others have their own exchanges, but the Native benefits are the same nationwide. 


 The Native protections are part of the ACA law, not tied to a single carrier. You can use them with any insurer offering Marketplace plans in your area, subject to availability and networks. All Marketplace carriers must honor zero cost-sharing for eligible AI/AN members up to 300% FPL.
